1. This special limited time offer (the “Offer”) is available to only primary enrollees and their dependents enrolled in select group plans administered by Delta Dental of California and affiliates (“Delta Dental”).
2. The Offer is limited to one Virtual Dental Screening per enrollee or dependent and is not transferable.
3. The Offer is not part of your existing Delta Dental plan benefits.
4. There is no cost or fee for the Offer.
5. Delta Dental may revise these terms and conditions at any time by updating this posting.
6. Delta Dental reserves the right to discontinue the Offer at any time without notice.
7. Virtual Dental Screenings provided through the Offer are not a substitute for regular dental exams and cleanings.
8. Any suggested further treatments or referrals that result from virtual dental screenings or the use of the Toothpic app will be subject to your Delta Dental plan benefits coverage.
9. Toothpic Independent Dentists are screened by Toothpic and not affiliated with Delta Dental’s provider network.
10. Toothpic is solely responsible for any service or information provided by Toothpic Independent Dentists.
11. Directories of Delta Dental network providers can be found online at www.deltadentalins.com.

Delta Dental will not be liable for any damages of any kind arising from the use of the Toothpic app, including, but not limited to, direct, indirect, special, incidental, exemplary, punitive, and consequential damages, whether in an action under contract, negligence or any other theory, arising out of or in connection with the use, inability to use, or performance of the Toothpic app, or any information, services, products and materials available on or via the Toothpic app.
